Intensive Family Support Practitioner

POSITION SUMMARY

Do you have the skills, experience and passion for working with families requiring support and empowerment in various areas of their lives?

Delivering an outreach model of family case-management programs to families, caregivers, kids and young people within Intensive Family Support Team.

Permanent part Time, Working 3 days per week, Wednesday, Thursday and Friday

Essential Skills and Abilities

  • A tertiary degree in a relevant discipline (allied health, social work, human services).

  • At least 2 years’ experience in similar position

  • Cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds, including Aboriginal and Torres Strait Islander people.

  • An understanding of current child protection legislation and policy reforms.

  • An ability to work collaboratively as part of a team.

  • Well-developed communication, problem solving and interpersonal skills.

  • Excellent IT skills in electronic database entry of case management information and using standard Microsoft Office applications including Microsoft Outlook or similar.

  • Excellent time management skills in maintaining a case load of clients, including timely record keeping and database entry, as well as meeting clinical supervision and other job requirements

Other requirements

  • Eligibility for a positive Working with Children Check in Queensland

  • Eligibility for an NDIS Workers Screening Clearance

  • A current driver’s licence.

  • Current CPR & FIrst Aid certification (or ability to obtain one)

Lifestyle Support Worker

POSITION SUMMARY:

Using a person-centred, client focussed support approach, the Lifestyle Support Worker will work as part of a team (which may include families) to provide support to service users with diverse needs to participate in activities as prescribed by the service user’s personal plan/s and or as directed by the Lifestyle Support Coordinator.

 Lifestyle Support Workers provide consistent and appropriate support to service users in areas including physical, emotional, behavioural, social support, personal care and lifestyle planning as agreed and detailed on each individual service agreement and personal support plan and provide opportunities for choice, self-advocacy and social inclusion. Lifestyle Support Workers will ensure the maintenance of each individuals rights, support appropriate outcomes for all individuals and be committed to best practice and quality service delivery

ESSENTIAL SKILLS AND ABILITIES REQUIRED:

Youth Worker : Casual

The Youth Worker will be an inspiring Team Player, encouraging youth to live their best lives, being active contributors in their community.

The YW position works within a dedicated team using collaboration to provide co-designed, holistic programs for youth within the community of Douglas.

 

SELECTION CRITERIA

  • Experience in working with young people communicating and engaging while helping to develop and deliver youth initiatives and events

  • Understanding of Culturally and Linguistically Diverse Backgrounds as well as youth who identify as Aboriginal and Torres Strait Islanders and supporting youth at risk in their communities.

  • Ability to foster strong connections with youth, staff, key stakeholders, local services, community, and funding bodies.

  • Demonstrated ability to work within a team

  • Demonstrated ability to be able to establish and maintain productive working relationships and collaborative alliances with community groups, community agencies, and government departments

  • Current Drivers Licence.

  • Workers Screening Check

  • Senior First Aid

Mossman Support Services is a local multi-faceted Not for Profit community servicing people in the Douglas Shire for personal crisis support  through advice, referral, advocacy, and information as well as offering community based programs and events to empower people to become engaged active participants in their community, building on their strengths and support systems for positive, sustainable outcomes in their life. This in turn helps to build on a stronger community as a whole. 

 

REMUNERATION

 

  • Hours: Monday – Thursday between the hours of 10.00 – 6.00pm on a casual basis

  • Pay rate: As per applicable SCHADS award
